Attached is some information on chemo fog from an Oct'08 conference that you might find helpful.

It speaks about the possible causes and then speaks to the fact that you have difficulty retrieving information. It is there, and you later remember it, but for the moment, can't retrieve it. Most commonly names and numbers are forgotten.

A way to cope with this is to write things down. Associate names with words. Place things in a memory area (ie car keys etc) so you don't have to be looking for where things were put down. Instead of calculating in your head, you might need to write it down or use a calculator now. You have to find and explore methods to work around the fog.

It also speaks to other issues that can be treated that are not related directly to chemo fog, but may be the cause, which you can explore with your doctor to rule in or out.
